AHI offers a wide range of programs aimed at advancing equity, quality, access, and affordability of health and healthcare services across the Adirondack region. Through collaborations with healthcare providers and community organizations, AHI supports initiatives that focus on enhancing care delivery in rural communities. Our programs include managing population health initiatives, expanding rural health services, strengthening the available healthcare workforce, and increasing access to critical services.

Insurance Enrollment Assistance

Health Home Care management

AHI is a New York State-designated lead Health Home and serves all ages. A Health Home is not a place; it is a care management service that connects individuals to healthcare services and community resources and supports. By connecting high-risk Medicaid individuals to these resources and supports, we aim to reduce duplicate services, reduce emergency department visits and inpatient admissions, and lower costs, thus improving the health and wellbeing of lives throughout the North Country.

About AHI

Adirondack Health Institute (AHI), an independent non-profit Article 28 organization under New York State Department of Health regulations, was established in 1987. For thirty years the organization has supported hospitals, physician practices, behavioral health providers, community-based organizations, patients and others in our region in sharing our vision and mission of transforming health care and improving population health.

Our vision is that every individual in the region reaches their fullest potential and lives their healthiest possible life.

Need Help With Insurance Enrollment?

Since 2013, Adirondack Health Institute’s Enrollment Assistance Services and Education (EASE) program has helped thousands of New York State residents navigate their health insurance options through the NY State of Health Marketplace. Our certified Navigators and Application Counselors provide free, unbiased assistance to individuals, families, and small business owners, helping them understand their coverage options and find affordable health insurance plans. Whether you’re eligible for Medicaid, Child Health Plus, Essential Plan, or a Qualified Health Plan, our expert team is here to guide you through the enrollment process with in-person or phone appointments across our eight-county service region.

Need Help With Care Management?

AHI Health Home Care Management serves as a vital bridge connecting Medicaid members to essential health and community resources across the North Country. Our care management services coordinate with primary care providers, hospitals, and behavioral health organizations to support individuals managing chronic conditions, mental health needs, or complex medical situations. Through personalized support and advocacy, we help members access crucial services including medical care, mental health treatment, housing assistance, transportation, and more—all at no cost to participants. Our dedicated team works collaboratively to improve quality of life and health outcomes for both adults and children in our community.
