Monthly Attendance Form: Traditional Services

Child care providers that participate in the DC Child Care Subsidy Program are required to submit attendance by fifth business day of each month using the Office of the State Superintendent of Education (OSSE) Attendance Tracking System (OATS). This form should be used to submit attendance for traditional services.
