Adoptee’s Access to Sealed Adoption Records

New Adoption Law: Act 100

Beginning July 1, 2023, the following persons may request a certified copy of their original Vermont birth certificate from the Vermont Department of Health:

An adoptee 18 years of age or older.
A direct descendant, 18 years of age or older, of an adopted person who is deceased.
Parent or guardian of a direct descendant who is under age 18.

What is needed?

Completed application signed in the presence of a Notary Public.
Check or money order in the amount of $10.00 payable to Vermont Department of Health.
Copy of adoptee’s current birth certificate, and if deceased, a copy of the death certificate.
Mail to the address on the back of the application.

Order Vital Records

Order Copies of Vermont Vital Records

The Department of Health issues certified and noncertified copies of vital records for births, deaths, marriages, and divorces that took place in the state of Vermont.

Certified copies are printed on State engraved security paper with raised seal, costing $10 per certificate (plus a $2 processing fee if ordered online).
Noncertified copies are printed on plain white paper with a “noncertified copy” watermark and are available at no charge.

Birth and death records from January 1909 to present, marriages, civil unions, divorces, and dissolutions may be ordered from our office. Vital Records may also be ordered from the Vermont State Archives and Records Administration and from most Vermont Town/City Clerks.

Option One: Talk to your Town Clerk

If you are in Vermont, the fastest and cheapest way to get a copy of a vital record is to order it directly from your Town or City Clerk. They can issue both noncertified and certified copies of the following vital records:

Any birth or death certificate from January 1, 1909, to present.
Birth or death certificates from 1908 and earlier if filed in their office.
Marriage and civil unions certificates on file in their office.

If you're out of Vermont or not in a rush, you can order certified copies of vital records online. Click the button below to place your order online and pay with a credit card. There is a $2 online processing fee.

You can order the following vital records by mail:

Follow the instructions on the application you choose and mail them to the address stated on the application.

Order noncertified copies of vital event certificates

To order noncertified copies of Birth or Death Certificates from 1909 to present, marriage and divorce certificates from 2023 to present, or civil union dissolutions for all years, please email [email protected] and include the following information in your request:

Type of event (birth, death, marriage, etc.).
Name(s) of the individual(s) named on the certificate.
Date of the event

Vermont Vital Statistics

The Vermont Vital Statistics System monitors the following vital events in Vermont:

Births
Deaths
Marriages
Divorces and dissolutions
Fetal deaths and abortions

In addition, the Health Department receives summaries of Vermont resident births and deaths that occur in other states. This allows the department to produce statistics for vital events that occurred outside the state.

Electronic Death Registration System

About the Electronic Death Registration System (EDRS)

This web-based application allows funeral directors and physicians, physician assistants, naturopathic physicians and advanced practice registered nurses to submit death records at any time and from any location having internet access. The EDRS facilitates more timely and accurate reporting of deaths which helps loved ones heal. It also helps us better understand public health in Vermont.
